PHYSICAL THERAPIST – FULL TIME – ROCK CREEK OF OTTAWA, KS
 
Our dynamic team is seeking a dedicated and experienced outpatient physical therapist to join our team at Rock Creek. As an outpatient physical therapist, you will have the opportunity to work with a diverse patient population in a supportive and collaborative environment.
 
Responsibilities:
 
  •   Conduct comprehensive evaluations of patients to determine their physical therapy needs and develop individualized treatment plans.
  •   Provide evidence-based outpatient physical therapy services to patients with various musculoskeletal and neurological conditions.
  •     Implement therapeutic exercises, manual therapy techniques, and modalities to improve patients’ mobility, strength, and function.
  •      Monitor patients’ progress closely and adjust treatment plans as needed to ensure optimal outcomes.
  •       Educate patients and their families on therapeutic exercises and home exercise programs to facilitate continued progress outside of therapy sessions.
  •    Collaborate with interdisciplinary team members, including physicians, nurses, and other therapists, to coordinate patient care and achieve treatment goals.
  •   Maintain accurate and timely documentation of patient evaluations, progress notes, and treatment plans in accordance with facility policies and regulatory requirements.
 
Qualifications:
 
  •                 Graduate of an accredited physical therapy program.
  •                 Current state licensure as a physical therapist.
  •                 Minimum of  2 years of experience working as a physical therapist, preferably in an outpatient or skilled nursing facility setting.
  •                 Strong clinical skills in evaluating and treating patients with orthopedic and neurological conditions.
  •                 Excellent communication and interpersonal skills.
  •                 Ability to work independently as well as collaboratively within a multidisciplinary team.
  •                 Commitment to providing compassionate and patient-centered care.
