Abilities Care Approach Introductory training: (Intended for Nursing, ED, Activities, Social Services, CNA/RNA leads) What is Abilities Care Approach, and your role as IDT in bringing the program to life.

Time: 10:30am-12:00pm

“Stage Specific Rehabilitation for Patients with Neurocognitive Impairments” : Focusing on the clinical skills therapists need to work with patients with dementia (with a focus on post-acute rehab patients).

Time: 1:00pm – 4:30pm – Earn 3.5 CCUs/.35 CEUs

Abilities Care Approach Webinar Series

“Implementing the Abilities Care Approach to Enhance, Participation, Well-Being and Quality of Life in Long-Term Care Residents”
A 6-week webinar series focusing on designing effective maintenance program and training caregivers in implementation of interventions in the long-term care environment
This course is intended for SLP, OT, OTA
Getting Started Checklist:
● Review course materials from OT 5-hour Abilities Care course
● Use Slums or MOCA for eval –consider using Cognitive Assessment Tool to obtain an estimate of the Allen Cognitive Level.
● Familiarize self with Abilities Care Approach resources on the Portal
● Screen patients (see Abilities Care Approach Screening Form) and identify 3 patient (ideally one in level two, three, and four)
● Send out Life History Profiles for referrals to be evaluated/ plan to have completed from caregivers by end of week 1 (Social serves may be able to help with this)
● Consider setting up a “Resource Binder” with frequently used materials for Abilities Care Approach program: Codes, documentation, forms, and assessments all available on the portal.

Join Instructor Dominic DeLaquil, PT for “HeartPARC” – Cardiac Recovery Program The Heart Post Acute Rehab Center (HeartPARC) Program is a professionally supervised interdisciplinary Post-Acute Cardiac Program designed to assist patients in recovering from heart attacks, heart surgery, heart failure and per cutaneous coronary intervention (PCI) procedures such as stent placement and angioplasty. The Heart PARC Program provides a comprehensive interdisciplinary approach to exercise, counseling and education designed to support patients with cardiac disease in achieving optimal physical, psychological, and functional status within the confines of their disease. Earn 4 CCUs/.4 CEUs
