Job Responsibilities:

  • Conduct assessments and evaluations of patients with cognitive and ADL (Activities of Daily Living) challenges
  • Develop and implement customized treatment plans based on patient needs and clinical standards
  • Provide evidence-based therapeutic interventions, including ADL retraining, exercises, and assistive devices
  • Continuously monitor patient progress, adjusting treatment plans as necessary
  • Collaborate with a multidisciplinary team to deliver the best possible care
  • Educate and counsel patients, families, and caregivers to support patient recovery
  • Prepare comprehensive discharge summaries in compliance with regulatory requirements

Qualifications:

  • Valid and current Physical Therapist License in the state of Arizona
  • Open to PTs of all experience levels
  • Strong communication and collaboration skills

Schedule:

  • Full-time, Monday to Friday
  • 8-hour shifts (flexible hours available)
  • Occasional rotating weekends and on-call availability as needed

Qualifications: 
Licensed Occupational Therapy Assistant 
Prior experience in a skilled nursing setting is preferred, but new grads welcome!
