Job Responsibilities:
- Conduct assessments and evaluations of patients with cognitive and ADL (Activities of Daily Living) challenges
- Develop and implement customized treatment plans based on patient needs and clinical standards
- Provide evidence-based therapeutic interventions, including ADL retraining, exercises, and assistive devices
- Continuously monitor patient progress, adjusting treatment plans as necessary
- Collaborate with a multidisciplinary team to deliver the best possible care
- Educate and counsel patients, families, and caregivers to support patient recovery
- Prepare comprehensive discharge summaries in compliance with regulatory requirements
Qualifications:
- Valid and current Physical Therapist License in the state of Arizona
- Open to PTs of all experience levels
- Strong communication and collaboration skills
Schedule:
- Full-time, Monday to Friday
- 8-hour shifts (flexible hours available)
- Occasional rotating weekends and on-call availability as needed
