$1,500 RELOCATION BONUS
We are seeking a full time PT at South Mountain Post Acute Care located at Baseline and Central in Phoenix. As part of our team, you will enjoy a stable in-house environment and have the freedom to implement the programs that you feel will most benefit your patients. We have an awesome facility team who can offer the support and training needed to provide excellent programs such as:
- Specialty programs for Parkinson’s Disease and other neurological impairments,
- Abilities Care Approach,
- Alter G treadmill programs,
- Cardiac rehab, and many more!
We are a skilled nursing facility with long term care, post-acute rehab, and geriatric outpatient therapy services provided. This is a great opportunity to be part of building something great in Phoenix as well as having the opportunity to be a part of our Master Clinician Program!

Duties:
- Effectively screens/evaluates patients with physical functioning disorders and/or wound care and develops appropriate plan of care following all regulatory and clinical practice standards.
- Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders and/or wound care per regulatory and clinical practice requirements.
- Administers various procedures as part of the rehabilitation plan including but not limited to: manual techniques; ambulation; therapeutic exercises; modalities; use of supportive and assistive devices; wound care; etc.
- Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
- Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
