PHYSICAL THERAPIST – DESERT TERRACE HEALTHCARE – PHOENIX, AZ

We are seeking a full time PT to join our in-house therapy team at Desert Terrace Healthcare Center, a busy and dynamic skilled nursing facility near downtown Phoenix, AZ. Programs include neuro rehab, fall prevention programs, wheelchair positioning, abilities care (Dementia), functional maintenance, amputation education, and beginning stages of outpatient therapy. Qualifications:
Licensed Physical Therapist
 
Duties:
  • Screen or evaluate patients within 48 hours of physician referral
  • Develop effective treatment plans
  • Treat patients according to treatment plan established with physician
  • Supervise Physical Therapy Assistants and students in direct patient care and patient related activities, following state practice acts and adherence to facility Supervision Policy and Procedure Matrix
  • Communicate with supervision and other health team members regarding patient progress, problems and plans
  • Participate in inservice training program for other staff in facility
  • Document all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy
  • Act as clinical preceptor for affiliating PT or PTA schools
